I recognize and acknowledge that there are risks, including personal injury, to participants in the activities at the Game Time Kids Amazing Race Fun Day (“Program”). By checking the box below, I agree to be responsible for the conduct and actions of any child (a “Participant”) that I have signed up to participate in the Program, and I agree and do hereby waive and release Game Time Kids, Miles Friedman, Peter Friedman, Lisa Acker, and each of the Program co-counselors and assistants and their respective parents and/or legal guardians (collectively, “Program Personnel”) from and against any and all claims, demands, and liabilities related in any way to any occurrences or to any actions by Program Personnel or others, including, without limitation, other Participants, during the Program (“Claims”). I have read and fully understand the above waiver and release of all Claims.

Center, Illinois that your child is going to participate in (“Dodgeball League Program”). By checking the box below, I agree to be responsible for the conduct and actions of my child attending the Dodgeball League Program (“Participant”), and I agree and do hereby waive and release Game Time Kids, Miles Friedman, Peter Friedman, Lisa Acker, and each of the Program assistants (collectively, “Released Parties”) from and against any and all claims, demands, damages, and liabilities related in any way to any occurrences or to any actions during the Dodgeball Program by any of the Released Parties, the Participant, or others, including, without limitation, any other participants in the Dodgeball League Program (“Claims”). I have read and fully understand the above waiver and release of all Claims.
